The John Fahey Handbook, Vol. 1 by Claudio Guerrieri states: "This implied that he [Fahey regarded the 1965 promotional version of Riverboat's The Transfiguration of Blind Joe Death as Volume 4, to be followed by The Great San Bernardino Birthday Party. A month or so later this would change, and The Great San Bernardino Birthday Party would be called Volume 4."
